The company pays up to $400/month in premiums, for the employee only. They handed out two fee schedule sheets, one for Single insured, the other for Spouse/Child.
Up to 29 years old, cost was covered in full by the $400. To add a dependent was an additional $438.
At 39 years of age, cost was $44 and an additional $494 for one dependent.
At 49 years of age, individual fee was $217 plus $667 for a dependent.
At 59 years of age, $547 for the employee, $1018 for dependent.
At 62 years of age, $674 and $1124, respectively.
All of the older employees shit a brick.
